FormlyMD provides a digital workflow tool designed to assist healthcare providers and patients in completing, reviewing, and securely delivering medical documentation. FormlyMD is an administrative tool, not a healthcare provider. We do not provide medical advice, diagnosis, or treatment.
To the maximum extent permitted by Canadian law, FormlyMD, its founders, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages resulting from your use of the Platform. FormlyMD assumes no liability for clinical decisions made by physicians or the outcome of insurance/employer claims.
You are responsible for maintaining the confidentiality of your account credentials. You must immediately notify FormlyMD of any unauthorized use of your account or any other breach of security.
We reserve the right to modify or discontinue, temporarily or permanently, the Service (or any part thereof) with or without notice. We may also update these Terms from time to time.
