Information submitted through FormlyMD is used to support medical documentation requests, doctor review, secure document preparation, patient communication, payment authorization, audit logging, and operational support.

Consent and Patient-Initiated Workflows

Patients access FormlyMD through a secure intake link associated with a participating medical practice. Submitted information is used for the specific documentation workflow requested by the patient and reviewed by the assigned doctor.

Limiting Access

Doctor, patient, and administrative areas are separated by role. Sensitive case documents are not intended for public access or indexing. Administrative access is logged for accountability and support purposes.

Safeguards

FormlyMD uses secure transport, protected application access, encrypted infrastructure components where applicable, and audit trails to help protect sensitive information during intake, review, document generation, and delivery.

Accuracy and Corrections

Patients and doctors are expected to review information for accuracy. Where appropriate, users may request corrections or updates through the platform or by contacting support.
