FormlyMD helps patients submit medical documentation requests online and helps physicians review, complete, and return forms more efficiently. The platform supports the workflow, but the final review and approval remain with the healthcare professional.
Does FormlyMD replace a doctor? expand_more
No. FormlyMD is a documentation and workflow tool. It does not diagnose, treat, or independently approve medical requests. A licensed healthcare professional reviews the submitted information before any completed document is issued.
What types of documents can I submit? expand_more
Patients can upload medical leave forms, sick notes, insurance-related forms, employer forms, and other supported clinical documentation requests. Availability depends on the doctor or clinic reviewing the request.
How long does the review usually take? expand_more
Review time depends on the physician, clinic availability, case complexity, and whether priority processing was selected. Standard requests may take several business days. Priority requests may be reviewed sooner, but timing is not guaranteed until the doctor completes the review.

After submission, your uploaded documents and intake answers are securely processed and prepared for doctor review. Once your case is ready, the reviewing doctor can verify the information, make changes if needed, and complete the document.
Can I edit my information after submitting? expand_more
If your case has not yet been reviewed, you may contact support or your clinic to request a correction. Some details may require doctor or clinic review before they can be changed.
When am I charged? expand_more
In many cases, payment is first authorized when you submit your request. The charge may be captured later depending on the clinic workflow and whether the doctor completes the review. Your portal will show the current payment status for your case.
Where do I download my completed document? expand_more
Once the doctor completes your document, it will become available in your patient dashboard. You may also receive an email notification when the completed document is ready.

FormlyMD organizes patient-submitted information, uploaded forms, and AI-assisted draft suggestions so doctors can review documentation requests more quickly. The physician remains in control of the final document.
Are AI suggestions automatically sent to patients? expand_more
No. AI-assisted suggestions are draft support only. They must be reviewed, edited, approved, and completed by the doctor before a finalized document is issued.
Can a doctor edit the completed form before sending it? expand_more
Yes. The doctor can review the PDF, edit suggested text, add notes, sign where appropriate, and make final changes before completion.

Medical and administrative records may need to be retained for legal, regulatory, professional, or healthcare record-keeping reasons. Patients may request corrections or account access changes, but certain medical records may need to be retained even if portal access is disabled.
Is my uploaded information encrypted? expand_more
FormlyMD uses encryption and access controls to protect sensitive information. Some highly sensitive profile fields are encrypted at the application level, and uploaded documents are stored with controlled access.
